Department Description

Transplant Quality Program

 

The Transplant Quality Program supports continuous quality improvement, patient safety, and performance excellence within the Multi-Organ Transplant Program. The program is responsible for the collection, analysis, interpretation, and reporting of clinical and operational data to monitor program performance, evaluate outcomes, and identify opportunities for improvement.

 

Working in close collaboration with interdisciplinary clinical teams, the Transplant Quality Program leads and supports quality improvement initiatives aimed at enhancing the safety, effectiveness, and patient-centeredness of transplant care. Through the use of data-driven methodologies, performance measurement, and evidence-informed practices, the program contributes to improved patient outcomes, regulatory compliance, and the advancement of high-quality transplant services.

 

What we offer:

 

Rate of Pay: $32.33 per hour to $43.07 per hour

Status: Regular Full-Time

Hours of Work: 37.5 hours per week

Qualifications

  • Successful completion of a two (2) year College Diploma in Health Information Management, Business, or Computer Systems Technology required
  • Currently Certified in good standing with the Canadian Health Information Management Association (CHIMA) required
  • One (1) to three (3) years' recent and relevant experience required
  • Recent experience with data entry into the Ontario Organ and Tissue Donation System (OATS) database through Trillium Gift of Life Network (TGLN) preferred

  • Advanced proficiency in data collection and management
  • Detailed content knowledge and proficiency with a variety of provincial and national healthcare data sets
  • Ability to understand and work with associated tools and applications
  • Ability to prepare and analyze complex reports; optimize existing technology to automate current manual reporting processes for improved efficiency and effectiveness; create complex queries to expose trends within specified indicators
  • Demonstrated excellence in identifying data needs to support decision-making and in determining appropriate data sources
  • Ability to assess data analytically for quality and consistency
  • Ability to anticipate and respond to client needs in a timely and effective manner and keeps team members informed about issues
  • Strong time management and organizational skills
  • Demonstrated analytical thinking
  • Relationship management
  • Advanced proficiency in Microsoft Office skills
  • Coding and Microsoft SQL skills an asset
  • Knowledge of clinical criteria and systems (including anatomy, physiology, pathology, pharmacology, and terminology)
  • Knowledge of Canadian and/or American coding standards (ICD10 at a minimum)
  • Strong written and oral communication skills
  • High attention to detail
  • Demonstrated practice and commitment to the principles of patient and family centered care
  • Demonstrated practice and commitment to patient and staff safety at LHSC
  • Demonstrated practice and commitment to LHSC's vision, purpose and values
  • Demonstrated ability to attend work on a regular basis
