About the Position

The Registered Nurse (RN) provides front line client care, outreach, testing and health promotion, to clients living with, or are at risk of contracting the Hepatitis C virus. The Hep C program is a collaborative initiative between the London InterCommunity Health Centre, the Regional HIV/AIDS Connection, and Infectious Diseases Care specialists, at St. Joseph’s Hospital.

Working to a full scope of practice, the RN Works with a multi-disciplinary clinical care team supported by Physicians, Nurse Practitioners, Dietitians, Social Workers, Community Development Workers, and Outreach Workers to provide comprehensive primary care focusing on the needs of people who are significantly marginalized by poverty, addiction, mental health and housing issues. The RN will support individuals along the continuum of care from health promotion, to harm reduction.

The RN also participates in the development, implementation, monitoring and evaluation of programs, services and quality improvement initiatives for the communities served including coordinating specialized clinics, teaching harm-reduction strategies and conducting outreach visits to meet clients where they are at, which may include street outreach, shelter visits or visits to community partner spaces.

The successful candidate will act with professionalism and courtesy toward clients, the public and colleagues and will demonstrate knowledge and valuing of the client’s life situation.

This full-time position typically works Monday to Friday, 37.5 hours per week, with some evening and weekend flexibility required.

Required Qualifications:

• Post secondary diploma (RN) or degree (BScN) from a recognized College or University.
• Current registration with the College of Nurses of Ontario.
• Three to five years of nursing experience in a community setting or combination of community and hospital or public health setting.
• Experience with phlebotomy.
• Experience working with people who inject drugs or use substances.
• Excellent written and oral communication skills.
• Knowledge and experience with the Social Determinants of Health
• Ability to work with diverse communities with respect, sensitivity, creativity, innovation, cultural understanding, non-judgement

Preferred Qualifications:

• Experience in a primary care setting
• Experience with Electronic Medical Records (Telus Practice Solutions Suite currently in use)
• Strong computer skills, including the use of MS Office Products

Nice to Haves:

• French and other language skills are considered an asset in supporting the communities we serve
