POSITION DESCRIPTION

POSITION TITLE: REGISTERED PRACTICAL NURSE

IMMEDIATE SUPERVISOR: CLINICAL MANAGER

Summary of Function:

Is responsible for the provision and management of care for technologically dependent/medically fragile/complex care children. The Registered Practical Nurse shares responsibility for ensuring individuals receive holistic and appropriate care. Advanced specialized knowledge, skill and judgement in assessing, planning, implementing and evaluating health needs of these children is needed. Care is provided within the framework, standards and scope of practice set out by Parents of Technologically Dependent Children and the provincial regulatory body (College of Nurses of Ontario).

Delegation of Care:

Assists in providing overall Clinical Supervision for all workers on duty. This includes Developmental Service Workers, Personal Support Workers, Volunteers and Students from all these disciplines. Is accountable for collaboration, teaching and judgement when delegating and assessing ongoing provision of holistic care. Assumes “In Charge” role as needed on shifts.

Qualifications:

Education:

· Current Certificate of Competence RPN in Ontario

· Current and annual CPR certification

· Current CPI certification

Experience:

· Paediatric clinical experience preferred

· Community health experience preferred

· Palliative care experience/Level 1 preferred

Skills:

· Observation for health and safety of all clients and workers

· Assessment of clients/families for physical and psychosocial challenges

· Adapting parental care plans to respite environment

· Effectively communicate with families, clients and care team

· Focus on interpersonal relationship building

· Apply critical thinking to problem solve and adapt care

· Working collaboratively with families and interdisciplinary care team

· Ability to be self-directed and direct others

· Apply current evidenced based practice

SERVICE RESPONSIBILITIES

1. Clinical Practice

The RPN provides service consistent with the standards established by Parents of Technologically Dependent Children and the provincial regulatory body. The nursing process is utilized: assessment, planning implementation and evaluation.

a) Provides continuous, complete and relevant assessment of client/family to include physical, emotional, psychosocial and safety needs.

b) Develops a care plan with client/family based on nursing assessment/family goals and prescribed medical regime.

c) Implements and maintains nursing strategies or interventions according to the established plan and medical regime. Coordinates the play therapies established by the DSW with the health needs and safety needs of the client.

d) Communicates/updates all care givers on care needs and changing care needs. Determines care priorities in collaboration with all care givers on any given shift.

e) Performs technical nursing skills at a safe level and in accordance with nursing principles and PTDC policies and procedures.

f) Utilizes appropriate interviewing and communication skills to obtain information from client/family.

g) Describes and documents nursing processes clearly, concisely, accurately and appropriately according to PTDC routines and CNO Standards.

h) Describes, evaluates and documents client/family’s responses to care plan for the achievement of realistic client-centered outcomes and revises care plan as needed.

i) Demonstrates evidence of negotiation with client/family during delivery of care.

j) Demonstrates respect for client/family choices and decisions. Provides care according to CNO Standards for Ethics and Therapeutic Nurse-Client Relationships.

k) Provides a safe, stimulating and positive environment for clients and caregivers.

l) Utilizes appropriate teaching/learning principles and materials to provide teaching for clients, families, staff, students and volunteers.

m) Analyses and incorporates current research findings into clinical practice.

2. Resource Management

a) Provides cost-effective, organized, outcome directed care on an ongoing basis.

b) Identifies and utilizes tools and resources for care and manages these resources through regular maintenance of equipment etc. Assumes responsibility for finding/completing solutions or routines that affect general operations in the home.

c) Demonstrates effective time management, planning and organizing on shift care/work activities.

d) Delegates care appropriately.

e) Ensures completion of workload measurement tool as required.

3. Leadership

a) Works collaboratively within the multidisciplinary team and acts as a clinical resource person and mentor of care providers, staff, volunteers and students.

b) Promotes involvement of client/family in determining health and service needs.

c) Influences policy review and development in health care and standards of

nursing practice through involvement in PTDC, professional organizations and/or community organizations.

4. Quality and Risk Management

a) Articulates knowledge of components of quality and risk management.

b) Plans, implements, evaluates and adjusts care in accordance with client/family focused expectations.

c) Seeks input from client/family to assure that care/service meets expectations.

d) Functions as an effective team member.

e) Participates in decision-making and follows up to promote continuous quality improvement.

5. Professional Accountability

a) Functions in accordance with the Standards of Nursing Practice of the College of Nurses of Ontario.

b) Functions in accordance with the PTDC policies, procedures and strategic direction of PTDC.

c) Functions in accordance with provincial legislation.

d) Maintains competence through continuing education.

e) Collaborates with management staff and colleagues in the evaluation of own practice.

6. Health and Safety Accountability

a) Functions and reports in compliance with the Occupational Health and Safety legislation, regulations and PTDC policies and procedures.

b) Participates in health and safety training including WHMIS and applies this knowledge in the workplace.

c) Wears and uses any/all protective equipment or clothing provided by PTDC and does not interfere with protective devices so as to change or negate their functions.

d) Reports the absence of or defect in any equipment or protective device of which he/she is aware and which may endanger a client, another staff or him/herself.

e) Ensures all workplace incidents are reported in a timely manner in accordance with legislation and PTDC policies and procedures for workplace injuries and incidences.
