Job Description
Rehab First Inc. is a multi-disciplinary rehabilitation assessment and treatment provider, specializing in complex case management, occupational therapy, life care planning, physio therapy, speech-language therapy, social work counselling and rehabilitation therapy. Our clients are children, adolescents and adults who have serious injuries and illnesses such as brain injury, spinal cord injury, amputation, multiple orthopaedic injuries, post-traumatic stress disorder, mental health issues and many other problematic injuries and conditions. We are committed to helping clients rebuild their futures and reach their potential at home, work, school and in the community. This position is community-based. In a home care setting you will assist clients with routine activities of daily living and personal care.

Key Responsibilities:
- Adhere to the plan of care as determined by the manager
- Assist the client with personal care and hygiene and activities of daily living
- Plan and prepare nutritious meals as required
- Provide cognitive, social emotional and psychological support to the client and family
- Maintain client information confidential
- Collaborate with members of the health care team in meeting the need of the client
- Respond to client and family concerns and ensure appropriate action taken within decision making authority
- Graduate of Personal Support Worker program
- Experience caring for individuals with brain injuries is beneficial
- Must have demonstrated sound skills and knowledge of providing personal care duties
- Superior interpersonal relations, communication, problem-solving, and time management skills
- Demonstrated ability to work collaboratively as a member of a multidisciplinary team
- Strong computer, written and verbal communication skills
- Valid Driver’s license, access to reliable vehicle and ability to travel
- Current Standard First Aid Certificate
- Current CPR level C Certificate
