Job Description

Reporting to the Nursing Supervisor, the RN is responsible for the assessment and provision of professional nursing care to clients in the community setting. Working from home,they will communicate with PSW's providing direct care to Palliative clients in their homes directing care activities.

Shift Hours: Nights, able to work from home

Key Responsibilities

  • To assess clients’ status and develop/ revise care plans and treatment regimes , including health teaching accordingly;
  • To access clients’ safety status, including physical environment, make recommendations, implement interventions and advocate on client’s behalf to ensure client’s safety is maintained;
  • To document clearly and accurately using the appropriate Revera forms in accordance with CNO standards;
  • To maintain client confidentiality ;
  • To provide professional nursing care in accordance with regulatory body standards& guidelines, applicable legislation and Revera’s policies and processes;
  • To participate in clinical case conferences as required;
  • To maintain competency in nursing skills;
  • To communicate appropriate client information within the client’s interdisciplinary team to promote quality, client focused care;
  • To communicate any serious client or risk concerns immediately to the Nursing Supervisor;
  • To maintain an effective working relationship with all internal and external stakeholders;
  • To mentor new staff/ students as required;
  • To participate in in-service education;
  • To uphold and promote the organization’s values and philosophy relating particularly to ethics, morality, and integrity as set out in Revera’s Code of Conduct;
  • To complete all other tasks as assigned.

Qualifications

  • Current registration in good standing as a Registered Nurse with their provincial regulatory body
  • Minimum of 5 year’s experience in palliative care and advanced care planning in the community

    -Additional certification/training in palliative care such as the Comprehensive Advanced Care Palliative Education (CAPCE) or Learning Essential Approaches to Palliative Care (LEAP) among others

    -Ability to draw information from Personal Support Workers and think critically enabling appropriate decision making; anticipates needs and support required

    - Once selected to the team must complete minimum of 2 days technical training on use of the eShift application portal, including virtual interaction and supervision with the Personal Support Workers.

  • Experience working in the community setting preferred
  • Must possess strong written and oral communication, critical thinking, clinical knowledge, time management and customer focus skills
  • Ability to perform physical tasks necessary for the position, e.g., transferring, lifting client as required
  • Current Basic Cardiac Life Support required
  • Skills in use of computer and Microsoft Office applications
  • Current (within 6 months) Vulnerable Position/Sector Screening (VPS).
