Job Description
Scope of Position:
Under the direction and supervision of the Supervisor of Nursing and Personal Care and as a member of the interdisciplinary care team, to be responsible and accountable for the nursing process (assessment, planning, implementation and evaluation of the delivery of direct care) for the Resident, working in cooperation with the home physician(s) / Medical Director and the Health Care Team. To plan and supervise the daily personal care to the Residents using a person centered model, being aware of the whole persons’ emotional, physical, intellectual and spiritual needs.
Position Description:
- To supervise the delivery of the Resident care process (assessment, planning, implementation and evaluation) to ensure individualized Resident focused care and service is provided.
- To maintain accurate, confidential, complete documentation and medical / nursing records (electronically and manually) for each Resident and to safeguard Resident information at all times to ensure it is accessed only by authorized personnel.
- To administer treatments and prescribed medications as required and to maintain the medication system.
- To teach, delegate, assign and supervise Resident Care.
- To collaborate with Residents, physician, family and staff, the Residents’ needs and condition, the care and individualized services required and to evaluate the effectiveness of the Residents’ Plan of care regarding the services provided.
- To participate on various teams / committees as required and ensure representation at meetings as required.
- To complete employee incident reports and provide on-site first aid as required.
- To assume responsibility for replacing staff (all departments) in cases of absenteeism after normal hours of work.
- To assume responsibility of the home in the absence of on-site management (after normal hours of work)
- To supervise the standards of Resident care in the home areas.
- To participate as a team member in providing Resident centered care and service.
- To undertake all duties and responsibilities in compliance with Health and Safety standards, guidelines and legislation.
- To adhere to infection control policies, procedures, protocols, routines and additional practices as required.
- To participate in in-service educational opportunities and staff meetings.
- Participate in annual mandatory education as per MOHLTC regulations.
- To maintain an awareness at all times of emergency procedures, and participate in drills/exercises and associated educational opportunities.
- To be aware of and comply with all County and home policies and procedures.
- To maintain confidentiality at all times.
- To orientate new staff and student placements as required.
- Perform other duties as assigned
Requirements
Knowledge and Experience:
- Registered Nurse with a current Certificate of Competence with the College of Nurses of Ontario;
- Previous relevant experience in a long-term care setting an asset;
- Courses specific to gerontology and dementia studies an asset;
- Working knowledge of Ministry of Health and Long Term Care regulation standards, Long-Term Care Homes Act, 2007 and relevant legislation;
- Resident Assessment Instrument, Minimum Data Set (RAI MDS) an asset;
- Supervisory experience an asset;
- Computer skills – experience with a computerized Resident Documentation system an asset; must be able to apply basic functions of Corporate standard software;

- Ability to facilitate and adapt to changes in the long-term care environment
- Current Basic Cardiac Life Support (BCLS) certification
- Ability to perform Heimlich Maneuver
- Valid Ontario driver’s license and access to a reliable vehicle
- Computer expertise in corporate standard software (Microsoft Office) and department specific software to data search and entry and to create and modify word processed documents
- Must be aware of safe work practices as they relate to job responsibilities and work environment, and have the basic understanding of the Occupational Health and Safety Act
