Job Description
Rehab First Inc. is looking for an experienced Registered Nurse (Case Manager) to join our team as we continue our expansion in Southern Ontario. As a multidisciplinary disability management provider, we have a rich legacy of client-centered consulting and service delivery. Our professionals work as part of a community-based program, allowing for flexibility and effective one-on-one intervention. Reporting to the Director of Rehabilitation, the Case Manager will be responsible for discharge planning, coordinating rehabilitation and health services, facilitating medical evaluations and home modifications, being involved in employment and vocational rehabilitation, maintaining ongoing collaboration with other medical and rehabilitation professionals and maintaining involvement with a client until all rehabilitation goals are satisfied.
Because of the nature of the job, candidates for this position must have a valid driver’s license, access to a vehicle and be willing to travel daily.
Qualifications:
•Current certification with the College of Nurses of Ontario
•Demonstrated knowledge of rehabilitation issues.
•Superior interpersonal relations, communication, problem-solving, and time-management skills.
•Demonstrated ability to work collaboratively as a member of a multidisciplinary team.
•Clinical experience with head trauma and brain injury is a definite asset.
Though not a requirement, NIDMAR certification is also of interest
