Direct Care Worker (Case Manager)

This full-time role involves working in a residential setting supporting adults with developmental, physical, and behavioural challenges. The Direct Care Worker role includes, but is not limited to, case management, ongoing advocacy for individuals, and supporting all aspects of daily living.

Starting Wage: $26.74 per hour plus benefits

Direct Care Worker (Case Manager)

Qualifications:
- Able to establish therapeutic relationships on the basis of respect and trust
- Support individuals with daily living while pursuing leisure and recreational opportunities
- Ability to work independently in the delivery of services on shift, as well as within a team
- Ability to write clear, concise observations of behaviour and medical concerns
- Proven computer skills, Microsoft Office preferred
- Ability to use sound judgment and make independent decisions based on Policies & Procedures
- Experience and proven success with case management, advocating on behalf of individuals, and liaising with outside professionals and families
- Strong organizational and problem solving skills; attention to detail
- Strong communication skills – written and oral
- Demonstrated ability to work well as part of the team
- Valid Ontario (G) Driver’s Licence, current Police Records Check with Vulnerable Position Screening, current Standard First Aid and CPR Certificate, CPI Certificate
- Post Secondary in a related field or equivalent
- Minimum of two (2) years experience in residential services, supporting individuals with developmental disabilities and high medical needs inclusive of personal care
- Bilingualism is an asset
