Job Description

The County of Elgin, Homes and Senior Services has an exciting career opportunity for a Temporary Full-Time Resident Care Coordinator (Contract) position at Elgin Manor Long Term Care Home in St. Thomas, ON.

Reporting to the Manager of Resident Care, the Resident Care Coordinator is responsible for directing Nursing staff to ensure resident care needs are met and appropriately documented according to individualized care plans in a timely and appropriate manner.

Key Responsibilities
•    Directs nursing staff relative to following resident care plans, PCC and MDS-RAI.
•    Provides input to the MRC to complete performance reviews and/or counselling.
•    Ensures timely communication with residents and family/personal representatives relative to care issues, concerns and requests.
•    Conducts routine and special care conference meetings, including Resident Care conferences, to foster communications and relations with family members and residents.  
•    Assists in the implementation of policies and programs as directed by the MRC.
•    Coordinates the admission process for new residents to ensure care plan development and implementation that supports a seamless transition for the resident and family.
•    Monitors and evaluates the effectiveness of quality improvement processes related to direct resident care. Utilizes process improvement skills to ensure continuous quality improvements.
•    Inventories and orders nursing supplies ensuring par stock is maintained and managed within budget.
•    Liaises with service providers to ensure coordination of, and compliance with, resident care plans, i.e.; Social Worker, Spiritual Care, one to one volunteer assistance through VON Palliative Care Agreement, interpreters, etc.
•    Ensures the MDS-RAI is completed by registered nursing staff according to individualized resident care plans, as evidenced by resident satisfaction, routine audits and continuous quality improvements.
•    Ensures MDS-RAI and care plan audits are completed and corrections/submissions are completed within required timelines.
•    Provides critical analysis and interpretation of data at various levels of aggregation (resident, unit, facility, region, etc.)
•    Works in a safe manner and with the protective devices, measures and procedures required by the County and the Occupational Health and Safety Act and regulations.
•    Other duties as assigned.

Qualifications
•    Regulated Health professional with current Ontario certification and/or Health Care Leadership/Management Certificate.
•    A minimum of three (3) years supervisory experience preferably in geriatrics or long-term care.
•    Applied knowledge of the Long Term Care Act, MDS-RAI and provincial legislations.
•    Experience and application of Continuous Quality Improvement processes preferred.
•    Effective team facilitation/leadership skills are essential.
•    Professional and positive interpersonal, supervisory and organizational skills are a must.
•    Proficient in the use of computer programs including MDS-RAI, Point Click Care, Word, Excel and Outlook.
•    Ability and willingness to work a flexible schedule including weekends and evenings.

Remuneration:         $33.54 per hour

Effective:             October 24, 2016 to November 7, 2017 or until the original incumbent returns (whichever comes first)
