We have an opening for a full time HOME (Health Outreach Mobile Engagement) Program Coordinator. This position typically works 37.5 hours per week Monday to Friday, with some remote evening/weekend flexibility occasionally required to support clinic activities as scheduled. This position offers a unique opportunity to make a difference in the lives of those impacted by homelessness, mental illness, substance use, poverty, complex and/or chronic health conditions. The Coordinator works with community partners and the HOME Program operations staff to effectively and efficiently provide clinical and social services to highly marginalized individuals in our community. Reporting to the Director of Program Development and Complex Urban Health, the successful candidate will provide leadership to support the work of the HOME Operations team while working with community partners to ensure successful delivery of services.

The HOME Program was developed to improve the health outcomes and health equity of highly marginalized individuals by providing health services to people where they are: shelters, encampments, housing, and in other community settings. The program is a collaborative approach between the London InterCommunity Health Centre, Middlesex-London Paramedic Service, CMHA Thames Valley Addiction and Mental Health Services, Regional HIV/AIDS Connection, and London Cares Homeless Response Services. The inter-disciplinary team provides primary care, harm reduction, support with substance use, basic needs, and wrap-around care services at various sites across the city utilizing our mobile clinic vehicles. This healthcare program aims to engage individuals who face barriers to accessing traditional models of care in harm reduction, stabilizing healthcare, and comprehensive care services. Partners will work collaboratively from an anti-oppression, anti-racism, health equity framework anchored in a participant-led approach.

REQUIRED QUALIFICATIONS

• Ability to successfully engage with multiple community partners
• Demonstrated experience in team leadership, coordination, or scheduling
• Minimum of 5 years experience in the health care and/or social services sector
• Awareness and understanding of the social determinants of health
• Commitment to the underlying principles of harm reduction
• Ability to work with people of diverse backgrounds with an inclusive and anti-oppressive lens
• Excellent written and oral communication skills; ability to maintain complete and accurate records in accordance with Health Centre policies

PREFERRED SKILLS

• Experience with Electronic Medical Records (Telus Practice Solutions Suite currently in use)
• Strong computer skills, including the use of Microsoft Office
• French and other language skills are considered an asset in supporting the communities we serve
