Interdisciplinary Primary Care Collaboration
2 Full-Time Positions - Ongoing Contract (37.5 hours per week)
The IPC program is a collaboration between London InterCommunity Health Centre (LIHC) and Thames Valley Family Health Team (TVFHT) with a focus on increasing primary care access for those who are marginalized from health services such as people who are unhoused and people living in poverty. The IPC team will provide access to comprehensive primary care with interprofessional team members to facilitate connections to internal and external referrals. The primary care model will be connected to the broader integrated resources of the TVFHT and LIHC.

About the Position
Working to full scope, the RN provides front-line outreach as well as onsite nursing care to people who face barriers to accessing health care, and are significantly marginalized by poverty, substance use, mental health, and housing instability and/or deprivation.

Required Skills & Qualifications
- Postsecondary diploma (RN) or degree (BScN) from a recognized college or university
- Current registration with the College of Nurses of Ontario
- Minimum 2 years of nursing experience in a community setting or a combination of community and hospital/public health
- Awareness and understanding of the social determinants of health and health promotion
- Ability to practice from social justice, trauma and violence informed care, and harm reduction frameworks
- Clinical knowledge of mental health care, substance use, concurrent disorders, dual diagnoses, and application of current best practices, assessments and interventions
- Ability to work with people of diverse backgrounds with an inclusive and anti-oppressive lens
- Excellent written and oral communication / interpersonal skills
- Strong computer skills including the use of Office 365 and Electronic Medical Records (Accuro currently in use)
- Ability to travel to multiple sites during the work day
Preferred Skills & Qualifications
- Experience in mental health care and working with people experiencing substance use disorders and homelessness
- Experience in preventative health and episodic care
- Experience in street level outreach, assertive engagement and motivational interviewing skills an asset
- Demonstrated phlebotomy skills
- Comfortable working autonomously
- Wound care competency
- Experience with chronic disease management
- Group facilitation experience (in-person/virtual)
- French and other language skills are considered an asset in supporting the communities we serve
