Child and Adolescent Mental Health Care Program- Inpatient

 

The Social Service Worker is an integral member of the Child and Adolescent Mental Health inpatient interdisciplinary team. The role supports children, youth, and families through psychosocial assessment, practical and emotional support, discharge planning, service navigation, and coordination with internal and external partners. The role exists to help identify discharge needs early, reduce barriers to safe and timely transition from hospital to community, and improve continuity of care for patients with complex mental health and psychosocial needs.

 

The Social Service Worker facilitates connections with community organizations, schools, child welfare, developmental services, respite, mental health agencies, and other resources required to support a sustainable discharge plan.

 

Rate of Pay: $29.01 per hour - $36.25 per hour

 

Hours of Work: 37.5 hours per week

Qualifications

  • Completion of a 2 Year Social Services Worker Diploma required
  • Current registration with the Ontario College of Social Workers and Social Service Workers
  • Minimum six (6) months recent clinical experience in social services or hospital setting
  • 1-3 years experience working with children, youth, and families in mental health, hospital, child welfare, developmental services, school or community agency settings preferred
  • Demonstrated knowledge of relevant legislation in relation to the health care consent act and substitute decisions act
  • Current knowledge of relevant legislation in relation to the health care consent act and substitute decisions act
  • Knowledge of social service assessment, discharge planning, community navigation and referral processes preferred
  • Experience coordinating complex discharges and working with high-risk youth and families preferred
  • Knowledge of child and adolescent mental health systems, school board supports, CAS processes, respite and caregiver support pathways, youth-serving agencies, crisis services, trauma-informed and family-centred care preferred
  • Strong verbal and written communication skills
  • Strong interpersonal and facilitation skills
  • Demonstrated strong critical thinking and problem-solving skills
  • Demonstrated proficiency in Microsoft Office and familiarity with electronic health records
  • Possesses strong communication skills (both written and verbal)
  • Possesses strong presentation, interpersonal and facilitation skills
  • Demonstrated practice and commitment to the principles of patient and family centered care
  • Demonstrated practice and commitment to patient and staff safety at LHSC
  • Demonstrated practice and commitment to LHSC's vision, purpose and values
  • Demonstrated ability to attend work on a regular basis
