THE PROGRAM
Our mobile health services include a team of social services workers and clinical staff who partner on outreach to provide episodic care in the community. Travelling by SUV, on foot, or with our Clinic on Wheels, the team brings care and connection to London’s hardest to reach. This program is a collaborative effort between CMHA Thames Valley, London Cares, Regional HIV/AIDS Connection and London InterCommunity Health Centre.
THE ROLE
The Nurse Practitioner works as a member of an interdisciplinary care team providing episodic care, wound care, basic needs and service connection referrals. The clinical team is a mobile team that works in various areas in the city with individuals that have experienced barriers accessing care and services. The team has quick access to refer to in-house Social Workers, Outreach Workers, Community Developers and support staff specializing in the areas of Mental Health, Systems Navigation, Diabetes and Chronic Diseases while out in the community.

REQUIRED QUALIFICATIONS
- Bachelor or Masters of Science in Nursing from a recognized university
- Primary Health Care Nurse Practitioner Certificate
- Registered Nurse in the Extended Class in good standing with the Ontario College of Nurses
- Minimum of two years’ primary care experience
- Awareness and understanding of the social determinants of health
- Commitment to the underlying principles of harm reduction
- Ability to work with people of diverse backgrounds with an inclusive and anti-oppressive lens
PREFERRED SKILLS
- Experience in providing care in an outreach or episodic setting
- Experience with Electronic Medical Records (Telus Practice Solutions Suite currently in use)
- French and other language skills are considered an asset in supporting the communities we serve
