PASSPORT WORKER
INDEPENDENT CONTRACTOR
 
Family Service Thames Valley is growing their Passport Program.  We have the goal to support people in creating meaningful informal and formal support networks in their own communities.  Our focus is to enhance existing community networks and natural supports.  The Passport Program support is individualized, flexible, supportive, non-judgmental and voluntary.

Passport funding provides a variety of support options to fit the needs and goals of each person receiving support. It can be used to access community participation activities such as leisure and recreation, skill development, volunteer opportunities or other personal interests.

Role Description:
Under the direction of the Passport Service Coordinator, the Passport Support Worker will:
  • Provide individualized and flexible support for adults who have a developmental disability
  • Foster social, emotional and community participation skills
  • Support may include:
    • Developing work, volunteer and daily life skills
    • Providing information about continuing education and personal development
    • Providing support to access emergency, or short term community support services
    • Providing support to attend medical appointments, court proceedings, understanding information provided and processes involved
    • Engaging existing natural supports
    • Connecting and participating in leisure activities
  • Attend team meetings as scheduled
 
Qualifications:
  • DSW, or relevant post secondary education in Human Services Field
  • Excellent interpersonal as well as written and verbal communication skills
  • Experience in supporting people with developmental disabilities, as well as experience in supporting people with mental health issues is an asset
  • Self motivated, self directed, and organized
  • Knowledge of community resources
  • Ability to work independently and within a multifaceted team
  • Flexible schedule
  • CPR/First Aid and a current Police Vulnerable Sector check
