Cheshire is seeking a full-time Assessor
Cheshire is a non-profit organization that has dedicated over 36 years to supporting adults with physical disabilities and frail seniors to live independently in their communities. Through the provision of personal attendant services that are mutually respectful and value the contributions of all, Cheshire promotes a ‘Person Centred Support’ philosophy that empowers consumers by tailoring each individual’s care to their own unique needs, interests, habits and desires.

Reporting directly to the Director of Community Support, the Regional Assessor performs the assessment function for Cheshire and for CSS Agencies within the South West LHIN Region. This will be accomplished through the completion of consumer/client assessments using the interRAI Community Health Assessment and Screener instruments. Respecting the rights of consumers to participate in all decisions related to their personal care the Assessor completes comprehensive care plans, identifies service gaps and forwards referrals to local programs and services. The Assessor provides input and recommendations for system improvements and consults with Community Support Service agencies and system partners on emerging issues.
The Assessor is committed to living the Mission and Shared Visions of Cheshire Homes of London Inc. and is committed to consumer safety, on-going learning and continuous service quality improvement and continuity.

Compensation is based on a 35 hour work week and starts at $25.15/hr. Group Benefits, compensation for mileage and travel time will also be part of the total compensation.

- Possess a post-secondary school diploma/degree and a minimum three years’ experience with an in-home/community care organization or social/health service program
- Knowledge of the interRAI CHA instrument and skills using comprehensive assessment tools and protocols
- Knowledge of community resources available for individuals with disabilities and infirm seniors
- Knowledge of the regional health care system
- A valid Canadian driver’s licence and automobile insurance
- A negative and current Police Vulnerable Sector Check
- Advanced proficiency of Word, Excel, PowerPoint and Windows Explorer and databases
- Fluent in French and English (preferred)
