Job Description

The Immigrant and Ethno-Cultural team at London InterCommunity Health Centre is currently looking for a bilingual Community Health Worker who is passionate about working with seniors who face challenges related to chronic conditions that impact their health, wellbeing and ability to live in their own homes. This position will complement the work of the Seniors’ WrapAround Facilitators who develop individualized plans to support seniors and caregivers to stay in their homes.
 
This Community Health Worker is a frontline staff member committed to supporting positive change in the lives of francophone and immigrant seniors and adults.  This position will establish and maintain trusting relationships with the community and act as a liaison between clients and Health and Social services providers with a goal to facilitate access to services and improve the quality and cultural competence of service delivery.
 
The Community Health Worker works as a member of a multi-disciplinary team supported by physicians, nurse practitioners, nurses, social workers, dietitians, community development workers, wrap around facilitators and support staff.
 
Qualifications
  • Ability to communicate (oral and written) fluently in French, required;
  • Post-secondary education in Health Promotion / Sciences / Community Development or equivalent combination of education and applicable experience;
  • Minimum two (2) years of related experience in curriculum development and delivery;
  • Awareness of and understanding of the social determinants of health, health promotion and Adult Education Principles;
  • Knowledge of barriers to accessing health services faced by Francophone and Immigrant seniors and caregivers.
  • Experience developing and implementing outreach strategies to a culturally diverse senior community;
  • Ability to work with diverse communities with respect, sensitivity, creativity, innovation, cultural understanding, non-judgemental and facilitation skills;
  • Ability to maintain complete and accurate records in accordance with the Health Centre’s policies;
  • Strong computer skills, including the use of MS Office Products and Electronic Medical Records (Nightingale currently in use).
