Registered Social Worker Position
 

The Alzheimer Society London and Middlesex has been actively supporting families and individuals affected by Alzheimer’s disease and other dementias since 1982. We advocate for and provide support services, education and funding for research for those affected by Alzheimer’s disease and other dementias.  
 
Our mission is to provide hope, relief and support as we partner with individuals and families throughout their journey with dementia.   
 
Location: Alzheimer Society London & Middlesex, 435 Windermere Rd London  
Status: Full -Time, 35 hours per week; Monday – Friday; 8:30 am – 4:30 pm 
Immediate Supervisor: First Link Program Manager 
 
 About you:  You are an experienced social worker with demonstrated skills in clinical service provision. You have a passion to help others, especially those living with Alzheimer’s disease and other dementias (ADOD) and you are able to communicate with good judgment, sensitivity and tact. You enjoy working as part of a multi-disciplinary team and utilize your excellent interpersonal skills to ensure that clients receive the best, most comprehensive care. You are a capable clinician who can readily access the necessary internal and external resources your clients may need. You are a natural with computers and your written and verbal communication skills are first-rate.  You are an enthusiastic, highly organized individual with a demonstrated ability to deliver accuracy and attention to detail in your work.  You manage your time well and you are able to problem-solve through challenging situations and unexpected circumstances.   

Summary:  Under the direction of the First Link Program Manager, the incumbent will work with individuals and their families from London & Middlesex County who are affected by Alzheimer’s disease and other dementias (ADOD). The Registered Social Worker will be a part of the First Link program and will work with health and social service professionals from the Alzheimer Society London and Middlesex to ensure that community members living with dementia and their care partners have timely access to services and resources that are appropriate for them. The Registered Social Worker may provide interventions such as assessment, system navigation, short term one to one supportive counseling, crisis intervention, case management, discharge planning, and referrals to community agencies as appropriate.  

*Please Note: This position will be mainly 'work from home' for the time being as the COVID-19 situation continues (this will be monitored regularly).  Experience and comfort with 'Zoom' is an asset.  There may be some in-office time as well, which has been completely up to date with health and safety measure and protocols. 
 
Qualifications:  
• Minimum Bachelor of Social Work degree required;
• Eligibility for certification with the Ontario College of Certified Social Workers;  
• Experience providing integrated social work services;
• Extensive knowledge and understanding of Alzheimer’s disease and other dementias (ADOD); 
 • Experience working with persons living with dementia and/or their care partners would be an asset;
• Strong written and verbal communication skills;
• Ability and knowledge to work within a computerized environment;
• Valid driver’s license and access to a reliable vehicle (for home visits, community meetings and commuting to ASLM office);
• Ability to communicate in a language other than English would be an asset.
