WrapAround Facilitator

(Elgin County)

 

The WrapAround process exists to improve the lives of children and families with multiple complex needs. The philosophy of WrapAround encourages family choice, family ownership and family empowerment.


WrapAround is a team-based planning process intended to provide individualized, coordinated, family-driven care that will meet the complex needs of children and their families. Children that benefit from participating in the WrapAround process are often involved with several child and family serving systems (e.g. mental health, child welfare, juvenile justice, and special education). This can include children who are at risk of placement in residential based programs, and who experience emotional, behavioural, or mental health difficulties.


The WrapAround process requires that families, service providers, and key members of the family's social support network collaborate to build a creative plan that responds to the particular needs of the child and family. The WrapAround Facilitator is a key member of the team that develops the plan. Team members then implement the plan and continue to meet regularly to monitor progress and make adjustments to the plan as necessary.

The successful candidate will be based in our London office, he/she will facilitate the WrapAround process in Elgin County.

 

The ideal candidate will possess the following attributes:
 

  • A minimum of 5 years experience working in the area of children’s mental health and/or developmental services
  • Successful completion of WrapAround Facilitator training would be a definite asset
  • A strong understanding of, and commitment to the service coordination model and the principles of WrapAround
  • Demonstrated ability to work collaboratively with individuals, families and service agencies
  • Proven facilitation skills are an absolute necessity
  • Excellent interpersonal and written communication skills
  • Ability to work in an environment of change
  • Knowledge of and ability to access community resources
  • Proven ability to work both independently and collaboratively as a member of multidiscipline teams
  • University degree or advanced education and experience in social services

A valid Ontario Driver’s License, access to a reliable vehicle, and the willingness to travel and work flexible hours as required
