Summary of Duties

As a qualified member of the interdisciplinary team, and under the direction of the Director of Care (DOC), Assistant Director of Care (ADOC), and Registered Nurse (RN), the RPN/ Wound Care Coordinator assumes responsibility for a the overall delivery of wound care services to residents in the home.     

Work Performed

MAJOR DUTIES AND RESPONSIBILITIES:


Responsible for all skin and wound management related education delivery to all members of the interdisciplinary health team as needed.


Responsible for maintaining the integrity of the wound care program.


Responsible for assessing residents according to best practice, demonstrating clinical competence, and procedural skills – will require turning, lifting, and repositioning of residents for observation and assessment.


Responsible for obtaining and tracking signed delegation of application of treatment cream forms for all PSWs and HCAs.


Completion of the RAI-MDS coding and applicable RAPs for residents with stage II and above wounds.


Responsible for weekly audits of documentation related to the wound care program completed by the registered staff for consistency, accuracy, and completion.  


Will liaise with physicians and other interdisciplinary team members to create, transcribe, and build treatment orders and Care Plans.  


Responsible for chairing and leading the skin and wound management committee meeting.


Responsible for inputting wound statistics monthly for CQI meetings and identifying trends prior to the meeting.


Responsible for coaching and educating the registered staff in areas of skin and wound management and documentation standards.  As needed, will support and collaborate with registered staff for the completion of dressing changes and required documentation.  


Responsible for maintaining a current skin and wound list of all residents with stage II or greater wounds in the facility.  This list is to be distributed weekly to the DOC, ADOC and Dietician.  


Responsible for sending physicians referrals as required.


Responsible for completion of the annual Quality Protocol and program evaluation for skin and wound management.


Daily review and timely follow-up of wound care referrals.


Responsible for monitoring stock levels of wound care products.


Report all areas of concern or non-compliance related to the wound care program to the management team for follow-up.


Other Duties as assigned. 


INCONTINENCE DUTIES:


Lead the continence committee meetings and assist with the coordination of continence supplies and education.


Consult and work with continence supply representative to identify learning educational needs and communicate these to DOC/ ADOC in charge of education.


Completion of continence assessments upon admission and as needed for all residents


Develop and evaluate continence care plans for residents.


Weekly ordering of continence supplies using RHA profile forms, update weekly, or with significant change in resident needs.

Qualifications/Experience

Registered Practical Nursing Diploma.


Current Certificate of Competency with the College of Nurses of Ontario.


Certification of completion of a minimum 2 current, within the last 3 years, wound care courses (Canadian Association for Wound Care, R.N.A.O course, 3M, Global Wound Academy).


Minimum 2 years of experience working as a wound care nurse.


Proficiency in Point Click Care and RAI MDS.


Good interpersonal skills both verbal and written.


Current CPR certificate.

Compensation and Other Information

Start - $32.07/hr


6 months or 1040 hours - $32.45/hr


1 year or 2080 hours - $33.60/hr
