Job Description

Summary of Duties

 

Working under the supervision of the Registered Nurse or the Registered Practical Nurse and following the policies and procedures outlined by the home. The PSW provides personal care to residents to meet their physical, emotional, social, cultural and spiritual needs.

PSWs /HCA’s are active members of the interdisciplinary team, and assist in the planning, implementing and ongoing evaluation of individualized plans of care within the philosophy of care of the home.

 

Behavioural Supports Ontario (BSO) is a provincial initiative aimed at redesigning the system of care for older adults with complex and responsive behaviours associated with cognitive impairments, mental health and addictions, dementia, or other neurological conditions living in Long Term Care Homes and community settings.

 

Work Performed

 

The Behavioural Supports Ontario PSW will be a key point of contact within the

Dearness Home focused on residents experiencing or at risk of responsive behaviours.

 

The PSW will be responsible to: 

 
  • Provide consultation and behavioural support to individuals, and their families, with various types of dementia, delirium, mental health, addictions and other neurological conditions and those who have experienced acute behavioural change.

  • Assist staff in the Dearness Home to understand and develop effective approaches to provide care to residents who are experiencing or at risk of responsive behaviours.

  • Provide input into care planning and intervention for residents who are experiencing or at risk of responsive behaviours.

  • Work collaboratively with staff to ensure a supportive environment that enhances resident care.

  • Ensure the protection of resident rights and act in an advocacy role for the resident.

  • Provide education and support to staff, residents and families within the Dearness Home.

  • Work in an integrated manner with the Schedule 1 seniors mental health and addictions mobile teams, other specialist supports, and the BSO/ HCCSS (Home and Community Care Support Services) -wide project team.

Qualifications/Experience

 

Successful completion of a Personal Support Worker program that meets the

requirements under the Long Term Care Homes Act 2070 as well: 

 



• Current Basic Cardiac Life Support Certification or willingness to obtain.

• Experience in Long Term Care and knowledge of the Long Term Care Homes Act 2070 and Regulation 79/70.

• U-FIRST is a mandatory requirement, Crisis Prevention, Gentle Persuasive Approaches, Montessori Methods

Training OR other related training is an asset. 

• Two years recent experience working with individuals with various types of dementia, delirium, mental health, addictions and other neurological conditions and those who are experiencing or at risk of responsive behaviours.

• Exceptional interpersonal communication skills, both written and verbal.

• Demonstrated leadership, coaching, mentoring and problem solving skills.

• Proficient computer skills and experience with Microsoft Office Software.

• Demonstrated commitment to client-focused care supported by evidence-based clinical best practices.

• A caring empathetic attitude is crucial.

• Experience/familiarity with RAI documentation.

• Commitment to quality improvement and the ability to facilitate knowledge transfer.

• Current and clear criminal check from a Canadian Police Information Centre, including vulnerable sector check completed within the last six months.

 

Compensation Range

 

Salary

Start - $23.48 per hour

6 Months or 1040 hrs - $23.77 per hour

1 yr or 2080 hrs - $24.84 per hour

 

This posting is for (1) permanent part-time position. 

 

Other Information

 

Please Note: 

• This position will provide twenty four (24) hours of BSO schedule time per week.

• The BSO PSW hours will be scheduled during days (6am- 2pm), evenings (2pm-10pm) and nights (6pm-2am). Additionally overlapping shifts may be required (12pm-8pm). Scheduling may be subject to change dependent on need.

 

• The successful candidate may be required to attend training during normal daily

business hours. 

 

Weekend work may be mandatory.
