Job Description
Roles & Responsibilities
Client care management: Develop care plans based on best practices and holistic client assessment, monitor and provide client care, measure outcomes, and report/document, participate in client-focused teams with other health care providers.
Advocate for clients: Communicate client needs to health care stakeholders, work with the client, other agencies and sectors to identify and secure available resources, report abusive or unsafe conditions.
Referrals: Take action to coordinate or facilitate access to services within health and other sectors. Establish an inventory of community resources that can support appropriate planning.
Educate: Provide health teaching, illness and injury prevention with clients and families on complex health care issues, raise the awareness of community services and build client and family capacity promoting client independence and self-management. Participate in the advancement of home health nursing by mentoring nursing students and novice practitioners.
Communication: Utilize information technology, produce interdisciplinary reports, provide schedules for availability, and work collaboratively with members of the health care team. Build positive and constructive relationships with clients and families.
Qualifications Required
• RN, with current registration with provincial regulating body
• Knowledge of standards and guidelines of Enterostomal Therapy and wound care
• Technical knowledge of vacuum assisted closure therapy
• Comprehensive Enterostomal Therapy and wound care patient health assessment skills
• Thorough knowledge of wound care products and indications for use
• Completion of the Canadian Enterostomal Therapy (E.T.) Nursing program
• Current Driver’s License and reliable vehicle
• Experience in Home Health Care preferred but not required
